Mental Residual Functional Capacity Forms

An individual who is planning on applying for Social Security Disability benefits with a mental illness should have their treating doctor fill out a mental residual functional capacity (RFC) form before they submit their application. Mental illnesses can be difficult to validate without proper medical documentation. Having a treating doctor complete a mental RFC will significantly improve your chances for approval.
